A therapist, counselor or small practice needs a way for new people to reach out. The temptation is to ask everything up front: symptoms, history, insurance. A website contact form is usually the wrong place for that. A short first message is easier to complete and avoids collecting what you should not hold.
What the first form needs
Ask for name, a way to reply and a short message about what they are looking for. You might add a preferred contact method and a field for the best time. That is enough to start a conversation.
Add a line next to the form that says not to include sensitive health details in the message. Tell people what happens next, such as who will respond and when they can expect it.
Fields that work
- ✓Name — how to address them
- ✓Contact method — email or preferred way to reach
- ✓Short message — what they are looking for
- ✓Preferred time — when you can reach them
What to leave for your intake system
Medical history, diagnoses, insurance details and consent forms belong in whatever intake or records system your practice has chosen. Your website can link to that system after first contact. We do not build client areas or records systems.
Check your own professional rules and your intake vendor’s terms about what may be collected and how. We do not give legal or health privacy advice, and your counsel or professional body is the right source.
Words on the page that help
- ✓A short privacy note — approved by your counsel
- ✓A line about emergencies — tell people where to turn
- ✓Response expectations — when you will reply
- ✓Next step — what comes after the form
How we build it
We design the form with only the fields you pick, test that messages reach you and write the surrounding copy. You approve the wording, including any note from your counsel.
What to expect, honestly
- ✓We do not give legal or health privacy advice — consult your counsel
- ✓We do not build records or client areas — link to your own system
- ✓Form delivery is tested, not guaranteed forever — Website Care helps maintain it
