The junk removal customer is mid-chore. A garage that has become a storage unit, an estate to clear before a sale, a renovation pile on the curb. They want to know the cost range and how soon you can come, and they will not read much to find out.
Volume is the whole price
Junk is priced by how much space it fills in the truck. Ask in terms customers can picture: "a few items", "a quarter truck", "a full garage", "a whole house". Offer a short list of item types, since appliances, mattresses and yard waste are handled differently.
Add a question about where items are: curb, garage, upstairs, basement. Stairs change the labor.
What your form should ask

Say what you do not take
A list near the form of what you cannot haul saves calls: hazardous chemicals, paint, tires, if those apply to you. State it as your own policy. A friendly "we can point you to local disposal options" turns a no into goodwill.
Submissions come to your email with spam filtered, and the confirmation tells the visitor how quickly you reply.
